Dialysis Centers
The ESRD population is at high risk for drug-related problems and supports the role of a clinical pharmacists in this healthcare setting. Consultant pharmacists who care for these patients are required to manage an extensive list of medications.
The New Jersey Department of Health and Senior Services regulations for Chronic Dialysis Services require facilities to retain a consultant pharmacist who has no affiliation with the pharmacy provider of facility medications.
Services Include

Provide ongoing Medication Regimen Reviews (MRR).

Perform unit inspections.

Monitor and assist with all medication management policies, procedures, and survey plan.

Collaborate with staff on special projects and in-servicing.
